The attached document takes a very long time to load--over a minute on my system. When scrolling up and down LO regularly freezes while trying to draw the pages.

reproducible with LO 4.0.0.3 (Win7 Home, 64bit)

Opening the same file with MS Office 2007 takes only approx. 1 - 3 seconds.
In addition, there are a few smaller layout issues, but the checkboxes in the forms show errors.  Maybe these checkboxes are also the reason causing this long load time?

Just a rough investigation:
Throughout the document, there are 256 calls to https://secure.dcsf.gov.uk/ for retrieving the images hosted on the domain. 

Removing those images will help decreasing the load time of the doc to normal.

Indeed there are a lot of calls to secure.dcsf.gov.uk + certificates exchanges.
Since i know too little about certificates, I won't put any logs public.

Michael:  I wonder if a certificate cache could be possible and help here. If useful, I can send you console logs.

I have retested with 4.3.3 and 4.4.2-rc2. In both versions the document takes a long time to import. Once imported performance is fine.

Confirmed it takes a long time to load, adjusted title and severity.

Testing in a Linux LO master log build, file affecting the load time is:

<https://secure.dcsf.gov.uk/consultationsadmin/img/optionBox.gif>

due to:
'Server certificate verification failed: issuer is not trusted'
Trying to open the graphic file with a browser, Firefox displayed this:

'secure.dcsf.gov.uk uses an invalid security certificate. The certificate expired on 02/05/2013 12:59 AM. The current time is 03/11/2016 11:31 AM.'

Dates are in en-US format.

When the bug was reported the certificate was valid, but now it's no longer possible to reproduce the exact bug cause.

Without: <https://secure.dcsf.gov.uk/consultationsadmin/img/optionBox.gif> loading is still to slow.

tested under Win8.1 x64 and LibO 5.2.3.3 on a SSD disk
it takes almost 7 minutes and 30 seconds to load from double click

tested on win7   
LO 5.4.3.2 x64    2 min 15 seconds to load test document
LO 6.0.0.0beta1 x64 -   4 seconds to load (like any other *.doc in my system)

looks like the problem is solved

yes, just a few seconds to load in 6.0 beta1
